Type-two diabetes limb amputations rise dramatically

The number of people in England having a limb amputated because of type-two diabetes has risen dramatically, a study has shown.

Between 1996 and 2005, below-ankle amputations doubled to more than 2,000, and major amputations increased by 43%.

However the number of amputations in people with type-one diabetes dropped, an Imperial College London team found.

A charity said more early diagnosis was needed, as diabetes can go undetected for more than 10 years.

The number of people diagnosed with type-two diabetes had increased greatly in the past decade, which could partly explain the findings, the researchers said.

In the UK, 2.35 million people have type-two diabetes, which is caused by the body not properly responding to the insulin it produces, and is often a result of lifestyle factors such as obesity.

Along with complications such as heart attacks and strokes, people with diabetes are far more likely to develop foot problems, including ulcers, which can become infected and lead to gangrene.

Writing in the journal Diabetes Research and Clinical Practice, the researchers said the number of the most major amputations, above the ankle joint, had risen by 43%.

And the average age of those having above-ankle amputations fell from 71 to 69 years, which followed the pattern of people being diagnosed younger, they said.

More men underwent amputations than women, a gap that widened during the 10-year period.

Study leader Dr Eszter Vamos said they had expected to see long-term complications of diabetes rising to some extent because the number of people diagnosed with the condition had increased.

"But at the same time there is very strong evidence that with a multidisciplinary team approach you can prevent up to 80% of the amputations.

"It highlights the importance of frequent foot checks and that it is very important to get glycaemic control and blood pressure and cholesterol control."

Diabetes UK care manager Sara Spiers said there were about 100 diabetes-related amputations each week in the UK.

"The findings of this study are especially interesting because they indicate a particular rise in the number of type-two diabetes related amputations," she said.

"This rise could be for a number of reasons, including type-two diabetes not being diagnosed early enough, people not getting the education they need to manage their condition effectively, and swifter treatment of foot problems, meaning people are more likely to have an amputation than die because of foot ulcers."

She added more early diagnosis was needed, as the condition could go undetected for more than 10 years and most people already had complications when they were diagnosed.

Exercising together appears to increase the level of the feel-good endorphin hormones naturally released during physical exertion, a study suggests.

A team from Oxford University carried out tests on 12 rowers after a vigorous workout in a virtual boat.

Those who trained alone withstood less pain - a key measure of endorphins - than those who exercised together.

Writing in Biology Letters, the authors speculate these hormones may underpin an array of communal activities.

It has long been known that physical exertion releases endorphins and that these are responsible for the sometimes euphoric sensations experienced after exercising.

They have a protective effect against pain.

But researchers from Oxford University's Institute of Cognitive and Evolutionary Anthropology found this response was heightened by the synergistic effect of rowing together.

After 45 minutes of either rowing separately or in a team of six, the researchers measured their pain threshold by how long they could tolerate an inflated blood pressure cuff on the arm.

Exercise increased both groups' ability to tolerate pain, but the difference was significantly more pronounced among the team rowers.

This, they said, was a measure of an increased endorphin release.

As well as potentially improving performance in sport, the researchers speculated that this endorphin release may be the mechanism that underpins the sense of communal belonging that emerges from activities such as religious rituals, dancing or laughing.

"The results suggest that endorphin release is significantly greater in group training than in individual training even when power output, or physical exertion, remains constant," said lead author Emma Cohen.

"The exact features of group activity that generate this effect are unknown, but this study contributes to a growing body of evidence suggesting that synchronised, coordinated physical activity may be responsible."

Carole Seheult, a sport and exercise psychologist from the British Psychological Society, said the findings were entirely credible.

"Rowing is a sport which requires real team work and endorphins could well foster that process.

"But more generally we know from experience that exercising in groups is good for people at many levels, it's motivational, it's social. Groups sessions really do work."

Molecules and synapses cement memories, say scientists

US scientists believe they have uncovered one of the mechanisms that enables the brain to form memories.

Synapses - where brain cells connect with each other - have long been known to be the key site of information exchange and storage in the brain.

But researchers say they have now learnt how molecules at the site of the synapse behave to cement a memory.

It is hoped the research, published in Neuron, could aid the development of drugs for diseases like Alzheimer's.

The deteriorating health of the synapses is increasingly thought to be a feature of Alzheimer's, a disease in which short-term memory suffers before long-term recollections are affected.

A strong synapse is needed for cementing a memory, and this process involves making new proteins. But how exactly the body controls this process has not been clear.

Now scientists at the University of California Santa Barbara say their laboratory work on rats shows the production of proteins needed to cement memories can only happen when the RNA - the collection of molecules that take genetic messages from the nucleus to the rest of the cell - is switched on.

Until it is required, the RNA is paralysed by a "silencing" molecule - which itself contains proteins.

When an external signal comes in - for example when one sees something interesting or has an unusual experience - the silencing molecule fragments and the RNA is released.

Kenneth Kosik of the university's neuroscience research institute said: "One reason why this is interesting is that scientists have been perplexed for some time as to why, when synapses are strengthened, you have the degradation of proteins going on side by side with the synthesis of new proteins.

"So we have now resolved this paradox. We show that protein degradation and synthesis go hand in hand. The degradation permits the synthesis."

Identifying the proteins the brain needs in order to cement the memory could ultimately have benefits for those suffering from memory disorders.

Rebecca Wood, head of the Alzheimer's Research Trust, said: "Scientists say they have studied nerve cells in the laboratory and learnt more about how specific proteins may have a role in areas of the brain that transmit messages and help us store memories.

"This interesting development could give a greater understanding of the memory loss experienced by people with Alzheimer's and other forms of dementia and lead to new treatments."

The most recent projections suggest 115 million people across the globe will suffer from dementia by 2050.

Julie Williams, professor of psychological medicine at Cardiff University, said: "Our increasing understanding of genetic risk factors in Alzheimer's is pointing to the synapses so any new study in this area is welcome.

"Alzheimer's is a complicated disease and it is early days, but the health of synapses and their activity levels is becoming an important and interesting focus of research."

Special Hormone Turns off Human’s Need to Booze

But researchers believe lowering ghrelin, a hormone produced in the stomach, may halt the desire to drink alcohol, and over eat.

Ghrelin is also known to promote appetite.

For the study, appearing in The Proceedings of the National Academy of Sciences of the USA, found mice pumped full of ghrelin were more likely to consume alcohol, when given the choice between water and hooch.

On the flipside, mice injected with an antagonist to ghrelin resisted alcohol. Scientists believe futzing with ghrelin could be used to treat alcohol addiction.

Cervical cancer link to early sex

Having sex at an early age has been linked with double the risk of developing cervical cancer.

An investigation into why poorer women have a higher risk of the disease found they tended to have sex around four years earlier than more affluent women.

It had been thought that the disparity was due to low screening uptake in poorer areas, but the study found this was not the most important factor.

The latest findings are published in the British Journal of Cancer.

Although the difference in cervical cancer incidence between rich and poor - across the world - had been noted for many years, it was not clear why this is the case.

Especially as rates of infection with human papillomavirus (HPV) - the sexually transmitted infection linked with the vast majority of cervical cancers - seemed to be similar across all groups.

The study, by the International Agency for Research on Cancer, of nearly 20,000 women, confirmed that the higher rates of cervical cancer were not linked to higher HPV levels.

But what it did reveal is that the two-fold increased risk was largely explained by women from poorer backgrounds starting to have sex at a younger age.

The age at which a woman had her first baby was also an important factor.

Screening was found to have some effect on the level of risk.

But the number of sexual partners a woman has and smoking did not account for any of the difference.

Lag time

Study leader, Dr Silvia Franceschi, said the findings were not restricted to adolescence and the risk of cervical cancer was also higher in women who had their first sexual intercourse at 20 rather than 25 years.

"In our study, poorer women had become sexually active on average four years earlier.

"So they may have also been infected with HPV earlier, giving the virus more time to produce the long sequence of events that are needed for cancer development."

Dr Lesley Walker, director of cancer information at Cancer Research UK, said the study raised some interesting questions.

"Although women can be infected by HPV at any age, infections at a very young age may be especially dangerous as they have more time to cause damage that eventually leads to cancer.

"Importantly, the results back up the need for the HPV vaccination to be given in schools at an age before they start having sex, especially among girls in deprived areas."

Men who do not openly express their anger if they are unfairly treated at work double their risk of a heart attack, Swedish research suggests.

The researchers looked at 2,755 male employees in Stockholm who had not had a heart attack when the study began.

They were asked about how they coped with conflict at work, either with superiors or colleagues.

The researchers say their study shows a strong relationship between pent-up anger and heart disease.

Writing in the Journal of Epidemiology and Community Health, the researchers called the various strategies for keeping things bottled up, covert coping.

The men were asked what methods they adopted: whether they dealt with things head-on, whether they let things pass without saying anything, walked away from conflict, developed symptoms like headache or stomach ache or got into a bad temper at home.

Hospital registers

They were checked for smoking, drinking, physical activity, education, diabetes, job demands and their freedom to take decisions.

Their blood pressure, body mass index and cholesterol levels were measured and they were aged 41 on average at the start of the study between 1992 and 1995.

Details of whether any of the men subsequently had a heart attack or died as a result of heart disease in the period up to 2003 were gathered from national registers of hospital treatment and deaths.

Up to 2003, 47 of the 2,755 men had a heart attack or died from heart disease.

The men who coped by sometimes or often walking away or who often let things pass without saying anything, had double the risk of a heart attack or dying from serious heart disease compared to men who challenged and dealt with the situation head-on.

Developing a headache or stomach ache or getting into a bad temper at home, did not increase the risk of heart attack or heart disease.

The researchers believe that anger can produce physiological tensions if it is not released and that these lead to increases in blood pressure which eventually damage the cardiovascular system.

'Conflict situations'

Dr Constanze Leineweber, who led the study from the Stress Research Institute in Stockholm, said: "There has been research before pointing in this direction but the surprise is that the association between pent-up anger and heart disease was such a strong one.

"I think men can't help how they behave in conflict situations - it's not something they think about, it's just how they react instinctively.

"If you are smoking and don't exercise you would be much more conscious of the risk."

Judy O'Sullivan, senior cardiac nurse for the British Heart Foundation, said: "Stress itself is not a risk factor for heart and circulatory disease, but some people's responses to stress, such as smoking or overeating, can increase your risk.

"We all find different things stressful and symptoms of stress can vary, but the important thing is that we need to find ways of coping with it in our lives in a positive way, whether at work or home."
